What’s Included in the Forms

The new patient packet includes:

  • Personal and contact information

  • Health history and current symptoms

  • Auto injury or accident details (if applicable)

  • Consent for treatment

  • Financial and privacy policies (HIPAA acknowledgment)

 

Completing these documents thoroughly helps us understand your needs, medical background, and goals—so we can provide personalized, effective care right from day one.

If you’re unable to print or access the forms online, don’t worry. You can arrive 10–15 minutes early to fill them out in the office. Our friendly staff will be happy to help.

What to Bring With You

Along with your completed forms, please bring the following to your first appointment:

  • A valid photo ID

  • Insurance card (if applicable)

  • List of current medications or previous diagnoses

  • Any relevant X-rays, imaging reports, or test results

  • Police/accident report (for auto injuries)

 

Dressing in comfortable, flexible clothing is also recommended for ease of movement during your exam.
